Summary :
Combination therapy for type 2 diabetes should achieve a durable effect as well as targeting the dual defects of insulin resistance and beta cell dysfunction.

The insulin-sensitizing effects of glitazones result in less strain on the pancreatic beta cell. When combined with submaximal doses of metformin or sulfonylurea (SU), combination therapy with TZDs has been shown to provide a sustained and more robust glucose-lowering effect.

The TZD/metformin combination has a powerful effect on reducing insulin resistance and is effective in the early stages of type 2 diabetes when more endogenous insulin is still available. This combination is also associated with minimal hypoglycaemia and less weight gain. The SU/TZD combination offers the added benefit of lowered insulin resistance and improvement in beta cell function.
