Summary :
In this presentation Dr. Krauss talks about treatment options that are available for the management of the atherogenic dyslipidemia associated with insulin resistance.
This atherogenic dyslipidemia is characterised by high triglycerides, low HDL cholesterol and an increase in small dense LDL particles. Although pharmacological treatments are available to address these abnormalities, Dr. Krauss recalls attention to the importance of lifestyle modifications. Various dietary weight loss programs, for example have been shown to improve lipid parameters, however adherence to the diet is an important determining factor for outcomes (1). In the Diabetes Prevention Program, intensive lifestyle modification including diet and exercise was also shown to improve lipid parameters and other components of the metabolic syndrome (2).
Pharmacologic options for managing atherogenic dyslipidemia are the LDL-lowering drugs, for example statins, and the triglyceride-reducing, HDL-raising drugs such as niacin and fibrates. Dr. Krauss shows how these treatments impact on triglycerides, HDL cholesterol and LDL particle size, when used alone or in combination, in different patient groups including those with type 2 diabetes. Finally, the lipid effects of the insulin-sensitizing glitazone drugs are briefly discussed, and Dr. Krauss ends by talking about current guidelines for the management of atherogenic dyslipidemia of metabolic syndrome.
