Summary :
Diabetes and obesity are intimately linked, says Dr. Goguen. Not only does visceral obesity lead to insulin resistance and diabetes but some antidiabetic agents also cause weight gain. Would stopping weight gain or promoting weight loss reduce the risk of cardiovascular disease in these patients?
Of the several components of weight loss programs, Dr. Goguen focuses on pharmacological treatment with orlistat, sibutramine and rimonabant. It is important to recognize certain problems that exist with drug trials in obesity, including high drop-out rates, a lack of proof of reduction in cardiovascular disease and/or mortality, and reduced drug effectiveness without concomitant diet and exercise.
Dr. Goguen talks about the modes of action, contraindications, side effects and dosages of orlistat, sibutramine and rimonabant, as well as their effects on cardiovascular risk factors and body weight with a focus on studies in diabetic patients.
