Summary :
In this presentation Dr. Leiter reviews the major results of the PROactive (PROspective PioglitAzone Clinical Trial In MacroVascular Events) study presented at the recent European Association for the Study of Diabetes (EASD) meeting in Athens.
Taking into account findings from the UK Prospective Diabetes Study (UKPDS), the 2003 Canadian Diabetes Association clinical practice guidelines recommend that in overweight type 2 diabetic patients with mild to moderate hyperglycemia, initial therapy should be a biguanide followed by an insulin sensitizer, insulin secretagogue, insulin or an alpha-glucosidase inhibitor (1). The glitazones improve insulin resistance and therefore a number of abnormalities contributing to the development of atherosclerosis, and it has been of interest to know the impact of glitazone treatment on vascular risk.
The PROactive study prospectively looked at the effects of pioglitazone treatment on total mortality and macrovascular morbidity in high-risk type 2 diabetic patients (2). Over 5000 patients received either pioglitazone or placebo in addition to existing therapy. Dr. Leiter presents the study design, and outcome and safety data as they were presented at the EASD meeting, and discusses how these results relate to the current Canadian treatment guidelines.
