Summary :
Payers and insurers are increasingly recognizing the importance of the metabolic syndrome as a risk factor for cardiovascular (CV) events. Due to limited resources, modern health systems must develop a managerial approach to the condition of metabolic system in the framework of their global CV management priorities. Only recently payers have expanded their management programs to include primary prevention for high risk patients in addition to traditional secondary prevention of CV disease. Assuming insurers will be willing to expand their programs to include additional patients with even a lower level of risk, decisions must be made regarding candidate groups, such as prediabetes, prehypertension or others, to be included in their programs. These decisions will be made based upon the evidence of potential risks, prevalence or incidence of the conditions and on the potential effectiveness of preventive measures. In this regard, the current definition of prediabetes has more than doubled the prevalence of the population which would be candidate for intervention. Payers must proceed with caution not to compromise care of the population already included in ongoing preventive programs, such as diabetic patients, by diverting scarce resources. Finally evidence is still inconclusive regarding the effectiveness of population-based intervention in these conditions on reducing long term CV events and other adverse outcomes.
