Summary :
The results of the FIELD (Fenofibrate Intervention and Event Lowering in Diabetes) study were presented at the AHA meeting held in Dallas in November 2005. This was a randomised trial of the effects of fenofibrate on coronary morbidity and mortality in people with diabetes, and in this talk Prof. Davis presents the data and discusses its implications.
Fenofibrate reduces triglycerides, total and LDL cholesterol, and increases HDL cholesterol, and the objective of the FIELD study was to provide the first randomised evidence of the effects of fenofibrate on the risk of major coronary disease events (non-fatal myocardial infarction or CHD death) in patients with type 2 diabetes and not taking statin therapy at study entry. Almost 10,000 patients were randomised to fenofibrate or placebo, with a planned follow-up of at least 5 years or until there had been a minimum of 500 CHD deaths or non-fatal myocardial infarctions.
There was a non-significant relative risk reduction in the primary outcome (CHD death and non-fatal MI) in those assigned to fenofibrate therapy. The relative risk of non-fatal MI by itself was however significantly reduced. The relative risk of the main secondary endpoint, namely total CVD events, was also significantly reduced in the fenofibrate group - interestingly those on active therapy without prior CVD received significant benefit whereas those with prior CVD were at the same risk as those on placebo.
Prof. Davis discusses other secondary and tertiary outcomes, safety data, and how the effects of fenofibrate seemed to be modestly attenuated by differential statin use. In conclusion, says Prof. Davis, the results of the FIELD study are likely to be of particular importance for patients without previous CVD, and where both the prevention of non-fatal macrovascular events and microvascular complications of type 2 diabetes are judged important.
