Summary :
"Lowering the level of atherogenic lipoproteins in plasma is unquestionably the single most important step that can be taken to reduce the complications of vascular disease," says Dr. Sniderman. How should this be done and does measurement of LDL-C provide an accurate picture of risk in all cases?
The apoB value represents the total number of circulating atherogenic lipoproteins, and is more useful for risk assessment when increased numbers of small-dense LDL (containing less cholesterol) are present. According to an expert panel report published this year by Barter and colleagues, evidence from fundamental, epidemiological and clinical trial studies show that apoB is "superior to any of the cholesterol indices to recognize those at increased risk of vascular disease and to judge the adequacy of lipid-lowering therapy." (1). "The number of atherogenic particles on therapy is the determinant of the residual risk due to lipoproteins," says Dr. Sniderman.
Dr. Sniderman further discusses the value of apoB compared to non-HDL-C and triglycerides, and shows new data from the CARDS study indicating the on-treatment apoB as a more accurate predictor of outcome than LDL-C for the primary endpoint and coronary artery disease in type 2 diabetic patients.
