Summary : Islet beta-cell failure is a key factor in type 2 diabetes pathogenesis. It is progressive, and occurs in the context of islets compensating for insulin resistance.
Beta-cell failure occurs in “susceptible” islets, and the mechanisms involved are of the initiation and progression types.
The factors that make islets susceptible to dysfunction need to be discovered, especially those that are acquired; and the mechanisms that have initiation roles in beta-cell failure. Disease modifying agents for the beta-cell in type 2 diabetes are needed.
