Summary :
Long-term randomised controlled trials have provided good evidence for the relationship between glycaemia and the risk of vascular complications of diabetes.

The DCCT study in type 1 diabetic patients showed that intensive treatment was associated with significant improvements in microvascular outcomes. There was a low number of cardiovascular events, however fewer events were seen with intensive treatment.

Almost all the DCCT survivors joined the the epidemiological follow-up EDIC study, in which they were returned to usual care. Over time, a greater progression of albumin excretion rate was maintained in the conventionally treated patients, and in the previously intensively treated group there was maintenance of a lower rate of progression. The curves for cumulative incidence of any predefined cardiovascular outcome for the two groups separated further in the EDIC study.

In the UKPDS in type 2 diabetic patients, intensive treatment was associated with a significant 25 percent risk reduction for microvascular outcomes but a non-significant 16 percent reduction in the risk of myocardial infarction. According to post-study monitoring data that has been presented at international meetings, with an increased number of events this became a significant risk reduction.

Prof. Davis reviews the findings, discussing the absence of a glycaemic threshold for benefit, and the apparent legacy effect of good glycaemic control.
