Summary : Prediabetes is an old term first used officially by WHO as a retrospective diagnosis to describe the state of a person which preceded the diagnosis of diabetes in that person. A radical new definition was introduced in the United States recently to describe collectively people found to have IGT and/or IFG. In other words it describes those with a high risk of developing diabetes in the future and already showing a glycaemic abnormality. These would have in the past been included in the WHO "potential diabetes" group. Pragmatically this new usage of the term prediabetes may be useful in that it conveys a clear message. However there are possible problems. Thus those with a first degree family history of Type 2 diabetes will be at as high a risk but may not yet show any abnormality in glucose levels. Also not all those with IFG or IGT will develop diabetes- even without intervention- with all the problems that can come about from an incorrect label.
None the less the new use of the term is firmly embedded in the literature and what is needed now is absolute clarity about its meaning & perhaps some refinement to include all high risk individuals. WHO and perhaps IDF should also be asked to approve any agreed new definition.
