Summary :
In this presentation Dr. Rodd reviews new findings in various areas of pediatric endocrinology.
Radioactive iodine treatment has long been used in adults with Graves' disease but its role and safety for use in pediatric patients has been unclear. Dr. Rodd starts by reviewing a 36-year retrospective analysis of radioactive iodine treatment in young Graves' disease patients. About a hundred patients were followed, and the treatment was found to be safe and effective in the long-term (1).
“Nephrogenic syndrome of inappropriate antidiuresis" is a term that has been used to describe the syndrome of inappropriate antidiuretic hormone secretion (SIADH) seen in 2 male infant patients having undetectable arginine vasopressin levels and a mutated V2 vasopressin receptor gene (AVPR2), causing constitutive activation of the receptor (2). Interestingly, both patients had mutations in codon 137, also known to cause nephrogenic diabetes insipidus.
Next Dr. Rodd talks about puberty and sex hormones, focusing on an article by Grumbach describing events in the “mini puberty of infancy.”(3)
Turning to type 1 diabetes, the findings from an 18-month phase 2 trial of CD3 antibody treatment were recently published. This treatment was shown to preserve residual beta-cell function in patients with recent-onset type 1 diabetes (4). In another study, it was reported that heterozygous activating mutations in the gene encoding the Kir6.2 subunit of the ATP-sensitive potassium channel cause permanent neonatal diabetes, and that identification of the genetic cause of permanent neonatal diabetes may facilitate the treatment of this disease with sulfonylureas (5).
Lastly Dr. Rodd talks about the 11β-hydroxysteroid dehydrogenase enzymes in adrenal corticosteroid biosynthesis, and primary hyperparathyroidism in pediatric patients (6).
