Summary :
In this presentation Prof. Jonathan Shaw from the International Diabetes Institute in Melbourne, Australia gives a detailed account of the final results of the DREAM (Diabetes REduction Assessment with ramipril and rosiglitazone Medication) trial presented yesterday at the 42nd EASD meeting in Copenhagen.
There has been growing evidence to suggest that in addition to lifestyle, metformin and acarbose, ACE inhibitors and thiazolidinediones may also prevent diabetes. The DREAM trial was designed to answer 2 questions: does ramipril 15 mg daily prevent diabetes, and does rosiglitazone 8 mg daily prevent diabetes? This was a double-blind, randomized clinical trial with a 2-by-2 factorial design, which randomly assigned 5269 participants without cardiovascular disease but with impaired fasting glucose or impaired glucose tolerance to receive ramipril or placebo and rosiglitazone or placebo, with a median follow-up of 3 years.
Overall, the DREAM trial showed that rosiglitazone has a substantial benefit on prevention of diabetes and regression to normoglycaemia, and ramipril has a modest benefit on regression to normoglycaemia. The durability of the glycaemic effect of these drugs is currently being assessed in a washout phase, and these findings are expected to be available this December.
