Summary :
AIM: To investigate the prevalence of the Metabolic Syndrome (MS, NCEP criteria) and the various combinations of its components in comparison to the expected frequencies of by chance clustering of these components.
SUBJECTS AND METHODS: We studied 1833 subjects, 20-74 years, in 5 Mediterranean Countries. Blood pressure, waist circumference, fasting glucose and lipids were measured. The prevalence of the individual components of the MS and of all their possible combinations was measured. The expected frequencies of the above combinations, if the components coexisted merely by chance, were calculated.
RESULTS: Abnormal HDL is found in 75.2% of the population, blood pressure in 51.3%, waist circumference in 27.9%, triglycerides in 26.5%, glucose in 5.5%. The more frequently observed combination is that of abnormal waist, HDL and blood pressure (14.4%), while the less frequent is the combination of abnormal waist, triglycerides and glucose (1.1%). The combination of abnormal waist, triglycerides, HDL and blood pressure is observed in 5.8% of the population, while each of the other 4 component?s combinations in approximately 1% and the combination of all five components in 0.7%. The expected, by random clustering, prevalence of all possible combinations of the MS components was significantly lower than the actually observed. Furthermore, the observed prevalence of MS (27.2%) was significantly higher than the expected by calculation (24.0%, p=0.03).
CONCLUSIONS: The clustering of the components of the Metabolic Syndrome occurs with a frequency far beyond the expected only by chance, suggesting the existence of an underlying driving force (insulin resistance?) aggregating the components.
