Summary :
In this presentation Prof. Davis covers several topics presented at the ADA's 65th Annual Scientific Sessions.
In a session on late-breaking clinical trials, data were presented on statin therapy in patients with diabetes in the TNT (Treating to New Targets) study, on the efficacy and safety of the PPAR alpha/gamma activator muraglitazar, and on the effects of rimonabant in mostly overweight/obese patients with type 2 diabetes in the RIO (Rimonabant In Obesity) Diabetes trial. Prof. Davis reviews these data and discusses newer therapies such as GLP-1 analogues, DPP4 inhibitors, inhaled insulin, and a new transdermal insulin delivery system.
With regard to chronic complications, data from the DCCT/EDIC studies showed a reduced incidence of CVD events in intensively treated type 1 diabetic subjects compared to the conventionally treated group. Data from the UKPDS showed different variables to be associated with an increased incidence of neuropathy. Also presented were data on erectile dysfunction, foot/leg ulcers and amputations. In the Diabetes Prevention Program, a linear increase in the prevalence of retinopathy was observed after diagnosis of type 2 diabetes.
Prof. Davis discusses novel hypotheses challenging the traditional view of autoimmunity versus insulin resistance in type 1 and type 2 diabetes, respectively, and studies on the prevention/treatment of type 2 diabetes in youth.
The ACHOIS (Australasian Carbohydrate Intolerance Study) showed various benefits due to intensive management of women with GDM, and other data were presented suggesting benefits of metformin therapy in insulin resistant women, in terms of pregnancy outcomes.
Prof. Davis ends the presentation by discussing some topics of general interest, such as new genes for obesity, the effects of IL-6, insulin infusions and acute MI, and a new PPAR drug, metaglidasen.
