"A Simple Tool to Predict the Risk of Cardiovascular Events in Patients with Diabetes Mellitus based on the Acarbose Meta-Analysis (MERIA)"

Summary :
OBJECTIVES: Different tools for the estimation of individual risk of cardiovascular diseases (CVD) in patients with diabetes mellitus (DM) exist. Our aim was to develop a risk score to predict the risk of CVD in patients with DM. In addition the score can estimate the risk reduction following acarbose treatment.

METHODS: The MERIA study showed that treatment with acarbose can prevent cardiovascular disease in diabetic patients while most of them are already on intensive concomitant cardiovascular medication. We used data from this study to develop Cox regression models for the time to onset of CVD. The final model equation was externally validated using data from the DECODE study.

RESULTS: The CVD risk-score equation included the variables acarbose treatment, sex, age, BMI, duration of diabetes and the ratio of low density lipoproteins and high density lipoproteins. A summary measure of discriminatory performance (mean sensitivity or specificity) is the area under the ROC curve (AUC). The CVD risk-score yielded an AUC value of 0.76 on the MERIA sample and resulted in an AUC of 0.74 on the DECODE sample.

CONCLUSIONS: The proposed risk score is a simple and validated tool to estimate the individual risk of CVD. High-risk diabetes patients would benefit most from CVD prevention strategies. Moreover, the risk score can estimate the reduction in CVD risk following acarbose treatment.
